No provider and patient relationship
Reading this website, filling out a form, signing up for emails, or booking a consultation does not make you our patient. A provider and patient relationship begins only after you complete an evaluation with one of our licensed providers and we agree to take you on as a patient.
Emergencies
Do not use this website, our forms, email, or text messages for urgent or emergency needs. If you think you are having a medical emergency, call 911 or go to the nearest emergency room.
Treatments and results
We only recommend a treatment after reviewing your history, labs, and goals, and some patients will not be good candidates for every service. Some medications we prescribe may be used off-label or prepared by a licensed compounding pharmacy, and your provider will explain this before you begin. Results vary from person to person, and we cannot promise any specific outcome. Patient reviews on this site reflect individual experiences and are not a guarantee of your results.
Virtual visits
Virtual visits are available only where our providers are licensed and where the law allows. Services offered virtually may vary by state.
Booking, payment, and cancellations
We are a cash-pay practice. We accept major credit and debit cards, HSA and FSA cards, and financing through CareCredit. Prices, program details, and policies may change at any time. We may ask for a card on file to hold your appointment. Our cancellation and no-show policy is shared with you when you book, and you agree to it when you schedule.
