LL-37: Your Body’s Own Antimicrobial Peptide

LL-37 is a peptide your own body makes. It is part of your innate immune system, your first line of defense against germs. It is the only peptide of its kind in humans, and patients ask about it for immune support, gut health, and skin healing.

What is LL-37?

LL-37 belongs to a family of antimicrobial peptides called cathelicidins. White blood cells and the cells lining your skin, gut, and lungs release it when they sense infection or injury. Vitamin D helps turn on the gene that makes it.

How it works

  • Fights germs. LL-37 can break open the outer layer of many bacteria and some fungi and viruses.

  • Breaks up biofilms. Lab studies show it can disrupt biofilms, the protective layers some bacteria build.

  • Helps healing. It draws immune cells to an injury and supports new blood vessel growth in wounds.

What the research shows so far

Most of the research is from lab and animal studies. A small human trial found that LL-37 applied to hard-to-heal leg ulcers helped them heal faster. There is little human data on LL-37 injections for immune or gut health.

The other side of LL-37

LL-37 can also drive inflammation. High levels are linked to skin conditions such as rosacea and psoriasis and to some autoimmune conditions. That is one reason we do not use it casually.

What we check first

If you get sick often or have ongoing gut problems, we start by looking at vitamin D, iron, blood sugar, and gut testing such as a GI-MAP. Raising a low vitamin D level supports your body's own LL-37 production.

Is LL-37 right for you?

We never start a peptide based on a trend. We look at your health history, current medications, labs, blood pressure, and goals first. Sometimes a peptide fits your plan. Often the bigger wins come from sleep, nutrition, strength training, and hormone balance, and we will tell you that.

This article is for education only and is not medical advice. LL-37 is not FDA-approved. Rules for compounded peptides have changed several times in 2025 and 2026, so we only prescribe what can be legally prescribed and filled by a licensed pharmacy. Talk with your own provider before starting any peptide.
